Training Programs with a minimum of 250 or 500 hours are eligible to apply if they meet the Training Program Guidelines and Requirements set forth by ISMETA.
In addition to filling out the form below and making payment on this page:
- Download the ISMETA Training Program Application Form.
- Be sure to download the file as a Word document, not just preview it online to ensure proper formatting.
- This document is a fillable form. Fill it out by typing where the form says “Click here to enter text.”, clicking in the appropriate check boxes ☐,
- and finally signing and dating at the bottom upon completion. Be sure to save the file as you work on it.
- Email the completed form to info@ismeta.org.
- Fill out the online application below including payment.
- Our Professional Standards Committee will review your application and follow up with you as necessary.
